Dear 2014 Skills Center Graduate,

Each year the Skills Center contacts its graduates from the previous school year to find out whether they are employed and/or going to college/post-secondary school, are in the military, or are unemployed. This information is very important to the Skills Center by helping us to receive funding to improve our programs. It is also useful in providing information to the high schools that continue to serve and send students to us. 


There are two ways you can choose to complete the Graduate Follow-up Survey/Questionnaire. You can either:

1. Click here to go to the Graduate Follow-up Survey/Questionnaire and take it on-line via survey monkey. 
2. Or, you may complete it via the following document: Graduate Follow-up Survey/Questionnaire. Note: Once you open this form you must first save it in your own documents before completing it. After completing the form, please save it again and send it back via email to: [email protected] . Options other than email are to mail or fax the completed document; the address and fax number are located below.
